It is possible just to dynamically set the TO user which will receive the email.
You can create a custom notification as below:MyCustomNotification
To: $NOTIFY
Notification Tyoe: Email and Inbox
Subject: MySubject [Cover Page.Number]
Body: MyBody for [Cover Page.Number]and in your code send it as below:
boolean priority = true; session.sendNotification(iChange, "MyCustomNotification", Arrays.asList(new IUser[]{iuser1,iuser2}), priority, "My Comment");
The My Comment information will be available only in the history of the object iChange but the $Notify is dynamically populated with the two iUsers

As mentioned above, please find the part of Import/Export documentation about the reason of why it is not possible to clear a filled field with an empty value
Importing Blank Fields
When you use the Import Wizard, it cannot perform a destructive operation. Existing Agile data is always preserved, never destroyed. Therefore, you can’t import a blank value to a non-empty Agile field. The Import Wizard ignores blank source fields.- 2867 views

You can create a quick script in JAVA using Agile API.
The following code will iterate for all ITEMS and set to empty the field Page Three.FieldName.
You can implement your criteria or iterate a predefined list of object(for example by number)IQuery iQuery = (IQuery) session.createObject(IQuery.OBJECT_TYPE, "Items"); ITable resultTable = iQuery .execute(); Iterator iterator = resultTable.iterator(); while (iterator.hasNext()) { IRow currItemRow = (IRow) iterator.next(); IItem item = currItemRow.getReferent(); item.setValue("[Page Three.FieldName]", ""); }

Have a look there for official documentation (Agile 9.3.2)
http://docs.oracle.com/cd/E28664_19/otn/pdf/user/E28676_01.pdfAs mentioned by cloudccw, it is necessary to create a new Item and add an Attachment on it but the questionis too general to have a specific answer.
Have a look at chapter 10 Working with Attachments. It is more or less the same for all Agile 9.3.* versions- 2464 views
